Our provider will put your health needs first. We are proud to provide a high quality level of customer service, medical experience, as well as a commitment to health and wellness for all of our patients. Our goal is to make you feel better as quickly as possible.

Priority Family Medicine is an independently owned and operated clinic that started in our providers hometown of Plainview, and has expanded to Norfolk, so she will strive to ensure that you are satisfied with your care, and we will treat you like family.  Being private owned also gives us the ability to keep costs low and work with you, the patient.  

We are located at 410 W. Locust Ave in the heart of Downtown Plainview and at 1300 W Nebraska Ave in Norfolk, right on 13th street, 2 blocks north of Walgreens.

About our Provider

Our provider, Michelle Hammer, is Board Certified, a member of the American Nurses Association as well as the Nebraska Nurses association.  She graduated Summa Cum Laude from Kaplan University with her Bachelor and Master's degrees.   She attended Northeast Community College for her Associate's degree in Nursing.   She has been awarded the "Top Advanced Practice Registered Nurse" by Today's Nurse.  She owns and operates PFM.  In her spare time, she is with her family camping, riding her motorcycle, or just enjoying the outdoors in general.  

Being a private owned clinic we offer a more personal level of care and we do so at non-corporate costs.  Being privately owned gives us the freedom to spend more time one on one with you, the patient, and also allows us to provide this at lower costs.
